I certify that the facts contained in this application (and accompanying resume, if present) are true and complete to the best of my knowledge. I understand that any false statement, omission, or misrepresentation on this application is sufficient cause for refusal to hire or dismissal if I have been employed, no matter when discovered by Hospitality of Bluegrass.
I expressly authorize, without reservation, Hospitality of Bluegrass, its representatives, employees or agents to contact and obtain information from any and all references (personal and professional), former employers, public agencies, licensing authorities and educational institutions and to otherwise verify the accuracy of all information provided by me in this application, and I release the employer and such other entities and individuals from any liability for any damages whatsoever that may result from their do doing. I understand that this application remains current for only 90 days. At the conclusion of that time, if I have not heard from the employer and still wish to be considered for employment, it will be necessary to reapply and fill out a new application.
I understand and agree that nothing contained in this application, or conveyed during any interview, is intended to create any employment contract. I further understand and agree that if I am hired, my employment will be “at will” and without fixed term and may be terminated at any time with or without prior notice, at the option of either Hospitality of Bluegrass or myself. No promises regarding employment have been made to me and I understand that no such promise or guarantee is binding upon Hospitality of Bluegrass unless made in writing.
If I am offered employment, I agree to submit to a drug test and criminal/credit background check before starting work or in the event of an injury while at work. If employed, I also agree to a drug test at any time deemed appropriate by Hospitality of Bluegrass and as permitted by law. I understand that my employment or continued employment, to the extent permitted by law, is contingent upon satisfactory drug test and background check, and if I am hired, a condition of my employment will be that I abide by Hospitality of Bluegrass’s drug and alcohol policy which may be amended by Hospitality of Bluegrass in whole or in part at any time.
I understand that filling out this form does not indicate that there is a position open and does not obligate Hospitality of Bluegrass to hire.
